SUMMARY
Parents are encouraged to read the Berwick Handbook. All forms must be returned prior to the boy attending the winter or summer program.

Request Info
Anyone interested in learning more about our organization, the winter program, or summer camp program should fill out and submit the Request Info form.

Boy’s Form
Interested boys should fill out the Boy’s Application Form. This will provide us with pertinent information needed when a boy joins our program. All boys must have this registration form on file prior to joining our programs.

Parent’s Form
Interested parents/guardians should fill out the Parent’s Application Form. This will provide us with pertinent information needed when their son joins our program. All parents/guardians must have this registration form on file prior to their son joining our programs.

Summer Permission/Release Form
Parents should fill out this Summer Permission/Release Form allowing your son to participate in our summer camp in Maine. This form will also allow us to use your son’s picture, or other likeness, in any publicity and/or campaign materials.

Winter Permission/Release Form
Parents should fill out this Winter Permission/Release Form allowing your son to participate in our winter program in Massachusetts. This form will also allow us to use your son’s picture, or other likeness, in any publicity and/or campaign materials.

Medical Forms
2 medical forms are required before camp starts.  A signed medical form needs to be filled out by the boy’s physician and returned to us prior to attending the summer program.  Also, a Camper Health History needs to be filled out by the parent or guardian.  Both of these forms are produced by the American Camp Association and the forms are linked from their website.  We are grateful to the ACA for making them available.

If the camper needs to self administer and drug for any reason, please complete the Permission to Self Administer Emergency Medication form that needs to be signed by the parent or guardian as well as the camper’s physician.  An example of this is the use of an “EpiPen.”

Berwick Handbook
The 2024 handbook is everything you want to know about our summer program in Maine. It also provides some background to the winter program and our overall organization. Parents and boys are strongly encouraged to read through the handbook.  Both parents and boys are required to sign the handbook’s Acknowledgement of Receipt Form prior to their son joining our programs.

Clothing/Equipment List
The Clothing/Equipment List  includes all items that the boys should and should not bring to summer camp.
