SUMMARY<\/strong>
\nParents are encouraged to read the Berwick Handbook. All forms must be returned prior to the boy attending the winter or summer program.<\/p>\n

Request Info<\/strong><\/a>
\nAnyone interested in learning more about our organization, the winter program, or summer camp program should fill out and submit the Request Info<\/em><\/a> form.<\/p>\n

Boy’s Form<\/strong><\/a>
\nInterested boys should fill out the Boy’s Application Form<\/a>.<\/em> This will provide us with pertinent information needed when a boy joins our program.\u00a0All boys must have this registration form on file prior to joining our programs.<\/p>\n

Parent’s Form<\/strong><\/a>
\nInterested parents\/guardians should fill out the Parent’s Application Form<\/a>.<\/em> This will provide us with pertinent information needed when their son joins our program.\u00a0All parents\/guardians must have this registration form on file prior to their son joining our programs.<\/p>\n

Summer Permission\/Release Form<\/strong><\/a>
\nParents should fill out this Summer\u00a0Permission\/Release Form<\/a>\u00a0<\/em>allowing your son to participate in our summer camp in Maine. This form will also allow us to use your son’s picture, or other likeness, in any publicity and\/or campaign materials.<\/p>\n

Winter Permission\/Release Form<\/strong><\/a>
\nParents should fill out this Winter\u00a0Permission\/Release Form<\/a>\u00a0<\/em>allowing your son to participate in our winter program in Massachusetts. This form will also allow us to use your son’s picture, or other likeness, in any publicity and\/or campaign materials.<\/p>\n

Medical Forms<\/strong>
\n2 medical forms are required before camp starts.\u00a0 A signed medical form<\/em>\u00a0needs<\/a> to be filled out by the boy’s physician and returned to us prior to attending the summer program.\u00a0 Also, a Camper Health History<\/a> needs to be filled out by the parent or guardian.\u00a0 Both of these forms are produced by the American Camp Association and the forms are linked from their website.\u00a0 We are grateful to the ACA for making them available.<\/p>\n

If the camper needs to self administer and drug for any reason, please complete the Permission to Self Administer Emergency Medication form<\/a> that needs to be signed by the parent or guardian as well as the camper’s physician.\u00a0 An example of this is the use of an “EpiPen.”<\/p>\n

Berwick Handbook<\/strong><\/a>
\nThe 2026 handbook <\/a>i<\/em>s everything you want to know about our summer program in Maine. It also provides some background to the winter program and our overall organization. Parents and boys are strongly encouraged to read through the handbook. \u00a0Both parents and boys are required to sign the handbook’s Acknowledgement of Receipt Form<\/em> prior to their son joining our programs.<\/p>\n

Clothing\/Equipment List<\/strong><\/a>
\nThe Clothing\/Equipment List<\/em>\u00a0<\/a> includes all items that the boys should and should not bring to summer camp.<\/p>\n","protected":false},"excerpt":{"rendered":"
